Tried to resolve the issue with Cameta and rather than revolve the issue electronically they require a printed form to be faxed (which I don't have a scanner/fax machine) or snail mailed. Kind of ridiculous that somebody who is willing to take your money electronically won't process their customer service requests the same way.
